Physical/ Mental Capabilities:
- Independent with activities of daily living: Dressing, bathing, toileting, etc.
- Ambulatory: Independent with cane or with walker, able to walk to dining room for meals, etc. No wheelchairs.
- Oxygen: If ordered by a physician, resident must be able to charge canister and change to portable system as needed.
- Diabetes/Insulin/Injectables Medication: Need to be able to self-administer, no sliding scales. Nurse will be available during daytime hours M-F to pre-draw medications.
- Mental Health: Not in danger to themselves or others; chronic depression stable on medication; no smoking or alcohol allowed (Tobacco Free)
Admission Requirements:
- Application completed
- Medical report completed by personal primary care physician
- Appropriateness assessment completed by nurse
- Room availability
- Income determination/funding sources
